Overview:
About MS
Multiple sclerosis (MS) stands as the most prevalent disabling neurological disease among young adults, typically surfacing between the ages of 20 and 40. The disease involves an autoimmune assault on myelin, the protective sheath enveloping nerve fibers in the central nervous system (CNS), encompassing the brain, optic nerves, and spinal cord. Myelin disruption occurs due to a misguided attack by immune system cells, leading to the formation of distinctive scar tissue, or plaques, visible through magnetic resonance imaging (MRI).
MS manifests differently across individuals, with some experiencing mild symptoms and others facing a progressive deterioration resulting in increased disability over time. The course of MS is unpredictable, with periods of symptoms interspersed with stretches of inactivity and recovery. Although the disease is not typically fatal, its impact on the quality of life can vary significantly.
The symptoms and severity of MS hinge on the location and extent of plaques in the CNS. These can affect white matter, nerve cell bodies in gray matter, and optic nerves. The cerebral cortex, the outermost layer of the brain, may shrink in the progression of the disease, termed cortical atrophy.
There are four primary types of MS, each presenting distinct symptom progressions:
While there is no cure for MS, various treatments aim to alleviate symptoms, reduce relapses, and slow long-term disability progression. Corticosteroids, administered intravenously, swiftly suppress the immune system and alleviate inflammation during acute attacks. Plasma exchange becomes an option for severe flare-ups.
Disease-modifying treatments focus on modulating or suppressing inflammatory reactions and are more effective in the early stages. Injectable medications include beta interferon drugs and Glatiramer acetate. Infusion treatments involve Natalizumab, Ocrelizumab, Alemtuzumab, and Mitoxantrone. Oral treatments include Fingolimod, dimethyl fumarate, Teriflunomide, Cladribine, and Diroximel fumarate. Each of these treatments carries specific benefits and risks, and the choice depends on factors such as the stage of the disease and individual responses.
As research advances, new therapeutic avenues continue to emerge, offering hope for enhanced management and improved outcomes for individuals living with MS. Siponimod, a recently FDA-approved oral medication, shows promise in treating secondary-progressive MS, marking a significant stride in the ongoing pursuit of effective treatments for this complex neurological disorder.
